#1 Device Acceptance Forms (Required*)
- Sign the form in person (At the school at time of pickup*)
or - Fill out the acceptance document ahead of time by requesting the e-Signature form here.
or - Opt out of using a District Device by requesting the e-Signature form here
#2 Internet Access (Optional based on need)
- If you do not have reliable Internet access for your child and require this service, request here.
#3 Device Insurance (Optional based on choice)
- Please see the specific cost for each device being offered here (English | Spanish)
